If you want the page optimized for A4, you must opotimize your print template for that.
There's no timeout setting, but if generating the PDF reaches timeout, it may be a general problem that we'll have to look at. In that case please report this to our support department.

The PDF output is Letter format, regardless of template size?

Yes, the physical size of the document is fixed.

according to abcpdf, the size of the page can be controlled by mediabox properties.

True, but there's no way to define this in Dynamicweb. I have suggested this feature to a future release. Please let me know other wishes you may have:)

It's both.
It is the way DW has implemented the PDF component.
They don't set the page size, and default it's Letter, but in DK this should be A4
