Forms for data lists - No table created

A customer created a form in DM Form. To my suprise there is no table generated for storage.

 

Is there a way to regenerate the whole table based on the fields which the customer are already created in the form designer?

 

It is not a small form so do not ask me or the customer to create a new form.

FYI i looked at table DMFormSetting. It contains a record for the form which has a tablename specified which is just one word. This table does not exist in the database.

There is no way to regenerate the form.

 

It could be an issue with the DB user not having DDL permissions?

i don't know what the customer did. New forms are working. I guess we have to recreate the form manually :(

FYI

 

 The customer had another form without table. Now i am able to reproduce it. There is a support case "10040" created.

The problem start when an empty form is saved and then trying to add new fields. The table will not be created in this case.
