It looks like it's related to _parsed files - because we also get errors in frontend when the layout is modifed and dw need to re-generate the _parsed version that it can't write to the files.
We have made some further findings on this one:
When we rename a template, upload new template into the designs-folder and browse ?parse=true DW tries to delete the _parsed folder - maybe while it's writing .parsed files inside the folder and then the frontend crashed with Access Denied to writing the .parsed files.
When I run iisreset then the _parsed-folder disappears.

The problem TFS#29991 "Cannot rename folders under design" has now been fixed in 9.3+ versions
