Ever since the first release of eCommerce, we have been strugling with this. It common in Portugal and Spain to have different tax according to the regions (mostly islands). To work arounf it, we created countries that the client would not use and gave it the name we needed.
Would be nice if we could have this as standard, because if the client decides to start selling to more countries, we have a problem.
Regions are basically dealt as countries, since shipping methods/prices and taxes are different.
