Create user on form submit

Hi,

On our website https://istwente.azurewebsites.net/ we have 3 forms:

- Contactform (https://istwente.azurewebsites.net/istwente/contact)
- Newsletter form (https://istwente.azurewebsites.net/istwente)
- Application form (https://istwente.azurewebsites.net/istwente/admissions/how-to-apply/application-form)

In all the forms I made the same setting 'Create user on form submit), but on the application form it does not work.
Do you have any idea what I am doing wrong?

Did you mark the fields that is needed to create the user? And define the email field?

I marked E-mail and E-mail permission. What else is required?

It might actually also explain why you did not receive emails from that form...
